Notice of Privacy Practices
Effective Date: April 1, 2025
This notice describes how health information about you may be used and disclosed by Coord Health Inc. dba Coord Health, the affiliated medical groups listed at the end of this notice (the “Medical Groups”), and licensed providers engaged by Coord Health Inc. dba Coord Health or the Medical Groups (collectively “Coord Health”) and how you can get access to this information. Please review this notice carefully. Coord Health is committed to protecting the privacy and confidentiality of your personal information and health information.
OUR RESPONSIBILITIES
Coord Health is required by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) to keep your personal health information ("Protected Health Information" or “PHI”) confidential. This Notice describes the ways in which we may use and disclose health information about you and describes your rights and our obligations regarding the use and disclosure of that information.
USES AND DISCLOSURES
Your Protected Health Information may be used and disclosed for treatment, payment, to support healthcare operations, and other purposes permitted, authorized, or required by law. Coord Health may use and disclose your Protected Health Information for the following purposes:
TREATMENT
We may use or disclose your protected health information to provide medical, telehealth, therapy, and related services as part of our Coord Health product and service offerings. We may disclose information to doctors, therapists, or other healthcare providers involved in your care.
PAYMENT
We may use and disclose your protected health information so that we can receive payment for the services provided to you.
HEALTHCARE OPERATIONS
We may use and disclose your health information to support, operate, and improve Coord Health products and services. For example, we may use your Protected Health Information to review and/or improve the quality of our telehealth services or respond to patient inquiries. We may de-identify your Protected Health Information to remove personal information that identifies you in order to develop new products and/or services or to work with others who develop such products and/or services.
AI Scribe is a HIPAA-compliant platform that helps our team measure progress throughout treatment and make adjustments to treatment as needed. Our EMR platform "Healthie" respects the privacy of all users and will never sell any personally identifiable data collected with the scribe. To provide the best care and attention, our providers - within Healthie - may use the tool called "AI Scribe" to transcribe conversations and helps with notes. Your information is private and the providers review content for accuracy. You can tell our provider team to stop using it at any time during care.
BUSINESS ASSOCIATES
We may disclose your Protected Health Information to other companies or individuals (“Business Associates”) who provide services to us necessary to maintain business operations. Business Associates must protect your medical information and notify us of any improper disclosure.
PERSONAL REPRESENTATIVES
We may disclose Protected Health Information about you to an authorized personal representative, such as an attorney, administrator, executor, or other authorized person responsible for you or your estate.
PRESERVATION OF HEALTH AND SAFETY
We may disclose Protected Health Information to prevent or reduce the risk of a serious and imminent threat to your health, to a third party, or to the public.
COMMUNICATIONS ABOUT OUR PRODUCTS AND SERVICES
We may use and disclose your Protected Health Information to contact you about other Coord Health products and services which we believe may be of interest to you.
INFORMED CONSENT FOR E-MAIL / TEXT COMMUNICATION
I consent to receive services and communications from Coord Health Inc. via e-mail and/or text message. I understand that Coord Health Inc. will take reasonable steps to protect my privacy and comply with applicable privacy and HIPAA regulations when communicating with me.
I also understand that standard e-mail and text messaging may not be fully secure methods of communication. In particular, text messages used for scheduling and logistical purposes may not be HIPAA-compliant, and there is a risk that such communications could be accessed by unauthorized individuals.
I acknowledge and accept these risks and consent to receive communications in this manner.
Appointment Reminders, Reschedules and Cancellations
I understand that Coord Health Inc. can reach me any time to remind me of my appointments or let me know in case of any change about my appointments. And I also understand that Coord Health Inc. may use a third-party automated system to notify me about information regarding my care, including but not limited to: appointment reminders, confirmations, and cancellations.
Risks
I understand that transmitting information by e-mail has a number of risks that should be considered.
E-mail risks include, but are not limited to the following:

E-mail can be circulated, forwarded, and stored in numerous paper and electronic files.

E-mail can be immediately broadcast worldwide and be received by both intended and unintended recipients.

E-mail senders can misaddress e-mail.

E-mail can be more easily falsified than handwritten or signed documents.

Backup copies of e-mail may exist even after the sender or the recipient has deleted his or her copy.

Employers and on-line services have a right to archive and inspect e-mails/text transmitted through their systems.

E-mail can be intercepted, altered, forwarded, or used without authorization or detection.

E-mail can be used to introduce viruses into computer systems.

E-mail can be used as evidence in court.
I understand that because medical information is sensitive, e-mail should not be used for communications concerning diagnosis. I understand that is the responsibility of the client/patient to protect their password or other means of access to e-mail to protect confidentiality. Coord Health Inc. is not liable for breaches of confidentiality caused by the patient.
Contact Information Change
I accept that I am responsible of notifying Coord Health Inc. when my contact informations change.
Notification Preferences
I understand that Healthie sends e-mail and push notifications to keep me up to date on messages received from my provider, comments on journal logs, upcoming appointments, goals, and program modules available for your review. I understand that I can configure the e-mail and push notifications that I would like to receive across the web and mobile apps, from within my account. I understand, however, that certain account communication (including appointment reminders) is set by my provider, and likely cannot be disabled for my account.
LAW REQUIREMENTS, LAW ENFORCEMENT
We may disclose your Protected Health Information to law enforcement as required by law, court order, subpoena, or other legal processes.
RESEARCH
Occasionally, we may use or disclose your Protected Health Information for research purposes. Researchers accessing your information are bound by the same confidentiality requirements.

USES AND DISCLOSURES THAT REQUIRE YOUR AUTHORIZATION
We will ask for your written authorization before using or disclosing your Protected Health Information for purposes not described above. You may revoke your authorization in writing at any time except for disclosures that have already been acted upon or are required by law. Coord Health will not sell or give your identifying information to third-party marketers without your written permission. Psychotherapy notes will not be disclosed without your written permission, except as required by law.
YOUR RIGHTS REGARDING YOUR PROTECTED HEALTH INFORMATION
You have the following rights with respect to your Protected Health Information. To exercise any of these rights, please contact our Privacy Office using the contact information below.

Right to Access Your Protected Health Information: You or your authorized personal representative may inspect or obtain a copy of your Protected Health Information by submitting a written request.

Right to Request Confidential Communications: You may request that we communicate with you by alternative means or at alternative locations.

Right to Correct, Amend, or Update Information: You may request corrections to your Protected Health Information; however, we are not required to accept all amendments.

Right to an Accounting of Certain Disclosures: You may request a list of certain disclosures of your Protected Health Information made by us or our business associates for purposes other than treatment, payment, and healthcare operations, limited to the last six years.

Right to Request Restrictions: You may request restrictions on certain uses and disclosures of your Protected Health Information; however, we are not required to agree except for certain out-of-pocket payments.

Right to a Copy of this Notice: You may request a paper or electronic copy of this Notice.
INFORMATION BREACH NOTIFICATION
We are required to notify you of any breach of your unsecured Protected Health Information, unless a risk assessment shows a low probability of harm. Notification will be made no later than 60 days after discovery of the breach.
QUESTIONS AND COMPLAINTS
If you have questions or concerns about our privacy practices or your rights, please contact our Privacy Office.
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. No retaliatory action will be taken against you for filing a complaint.
REVISIONS TO THIS NOTICE
Coord Health reserves the right to change our privacy practices and this Notice at any time, as permitted by law. We will post changes on our website or notify you directly.
